REPORT ON CONDUCTING PUBLIC COLLECTION

Legal basis: art. 9 of the Act of 15 March 1933 on public fundraisers (Journal of Laws No. 22, item 162, as amended)

On 17 and 18 December 2011, in AS stores: Brzozowa, Leszczyńskiego, Nowodworska, Chodkiewicza, Gierczak and Kiermasz na Chodkiewicza shop, sweets were collected for foster children's homes and children under the care of hospices and associations dealing with the help of poor and sick children.

This collection was carried out by the Organizational Committee "Sławek i Przyjaciele" with the registered office in Bydgoszcz on the basis of the permit of the Mayor of Bydgoszcz 3138 of 4.11.2011.

As a result, 530 kg of sweets were collected, which were delivered to the following institutions:

• Center for Children and Youth Poorly Hearing and the Unhearing in Bydgoszcz - 37 kg
• Social Welfare Home for Children and Young People with Intellectual Disabilities in Grabiu - 71 kg
• Association "Child Safety" - 102 kg
• Primary School - Community Center in Śliwice - 25 kg
• Primary School No. 4 Community Center in Fordon - 40 kg
• ZS No. 11 in Bydgoszcz - 46 kg
• Foundation "Wiatrak" - 84 kg
• Salesian Youth Education Association - 60 kg
• Social Welfare Home in Kamień Krajeński for children with moderate, severe and profound intellectual disabilities - 65 kg

All costs of the public collection were incurred by the members of the "Sławek i Przyjaciele" Organizing Committee based in Bydgoszcz.
